Dental Benefits and Eligibility Requirements

Participants can choose between an Indemnity Dental Plan (no network restrictions) or a PPO Dental Plan (additional savings by going in-network). Although there are no child-only dental plans, your dependent children under the age of 26 are eligible on your plan. Plans are available for primary applicants aged 18 and up, their spouse and their children.
What is Dental Insurance?

There are two dental coverage plans* to choose from: Indemnity or PPO. You can choose from 3 levels of care per plan, including Independence 1000, Independence Ultra 1500 and Independence 1500, so you can get the one best suited to you and your family.
